Our Patients
Our goal at Art of Dentistry is to make your visit pleasant and relaxing. We also know your time is important and want you to have an efficient visit. Our high-tech, green, paperless office was designed with your well-being in mind. We want you to feel at home so come in, have some coffee, munch on a cookie, and try to relax within our friendly environment. WiFi is available for your convenience. Surf the web while sipping on some coffee and enjoying a fresh-baked cookie. After you make your first appointment, if you provide us with an email address, we will send you the new patient forms to fill out before coming in. You can also find a link to these forms below. If you are not able to, or choose not to fill these out beforehand, we would ask you to arrive 15 minutes before your scheduled appointment to allow ample time to complete these forms and be seen on time. We will need one form of Identification (ID card such as driver’s license) and your insurance card at every visit. Your initial exam will last approximately one hour. Your dentist will thoroughly examine your teeth and gums, specifically looking for any potential problems. Radiographs will be needed for proper diagnoses. If there are any signs of decay or other problems, your dentist will recommend treatment options and make notes of any conditions that may need future observation. Oral hygiene instructions will also be provided along with suggestions to help you care for your teeth.
INSURANCE
As a service to our patients, we accept and file claims to most major dental insurances, as a courtesy to our established patients. If you have a question about your insurance and its coverage, feel free to call us to discuss them. Please keep in mind that we will need your dental insurance information at least two days prior to your appointment in order to have enough time to check eligibility and benefits. If you do not have dental insurance, don’t worry. We will be happy to discuss your options with you. You will be glad to know we offer a cash payment incentive plan. For your convenience, we can also help you apply for CareCredit.
FINANCES
Payment is expected at the time of service. Please click on the link below to read our complete Financial Policy. We accept the following forms of payment: cash, personal checks, money orders, and major credit cards. We would also be happy to help you apply for CareCredit.

If you are a returning patient, and any of your personal information or medical history has changed, please print and complete these forms with your new information, and bring them with you on your next appointment. The HIPAA Consent Form should be filled out at least once a year.
